Administration

There are a number of administrative forms which must be completed soon after your arrival. Please have with you the following information: Current life insurance policies (company name, company address and policy number) and a copy of your Immunization Record. You will be vaccinated against a variety of diseases soon after your arrival if you cannot prove currency of immunization protection.

The most important document is the notarized Appointment Affidavit. This form MUST be signed and notarized PRIOR to commencing your travel to Kings Point. If you do not have this completed form with you when in-processing commences on Monday morning, then the government cannot reimburse you for your travel expenses. If you were provided an airline ticket for travel to Kings Point and do not have a notarized Appointment Affidavit, then YOU will have to reimburse the government.
