A) Valsalva
B) Circumference measurements of NECK, Waist and/or HIPS
- Report of Medical History (DD2807-1) to include: completion of all items
- Active Duty/Reserve Forces Dental Examination (DD2813):
A) Type II-class I or class II exam
- OSHA Respirator Medical Evaluation Questionnaire (OSHA Respirator)
- Ocular Examination - to include:
A) gross visual acuity
B) visual fields
C) tonometry
D) color sense (PIP of FALANT)
** Any individual with a visual acuity less than 20/20, must be referred to an ophthalmologist or optometrist for refraction **
Laboratory Studies/Diagnostic Studies
- HIV
- Blood type and Rh factor
- Complete Blood Count(CBC)
- Blood Chemistry- to include:LDL,HDL, and triglycerides
- G6PD
- Sickle Cell
- RPR/STS
- Urinalysis
- PPD(unless contraindicated)
- Chest X-Ray
- Electrocardiogram (EKG)
- Audiogram (500/1000/2000/3000/4000/6000 MHz)
- Spirometry
