Inter-Service Transfer Medical Requirements:
 
  • Report of Medical Examination (DD2808) to include:

A) Valsalva

B) Circumference measurements of NECK, Waist and/or HIPS

  • Report of Medical History (DD2807-1) to include: completion of all items
  • Active Duty/Reserve Forces Dental Examination (DD2813):

    A) Type II-class I or class II exam

  • OSHA Respirator Medical Evaluation Questionnaire (OSHA Respirator)
  • Ocular Examination - to include:

    A) gross visual acuity

    B) visual fields

    C) tonometry

    D) color sense (PIP of FALANT)

** Any individual with a visual acuity less than 20/20, must be referred to an ophthalmologist or optometrist for refraction **

Laboratory Studies/Diagnostic Studies

  • HIV
  • Blood type and Rh factor
  • Complete Blood Count(CBC)
  • Blood Chemistry- to include:LDL,HDL, and triglycerides
  • G6PD
  • Sickle Cell
  • RPR/STS
  • Urinalysis
  • PPD(unless contraindicated)
  • Chest X-Ray
  • Electrocardiogram (EKG)
  • Audiogram (500/1000/2000/3000/4000/6000 MHz)
  • Spirometry

Female Candidates:

  • PAP Smear
  • Pregnancy Test

NOTE: Laboratory and Diagnostic test results must be submitted with the physical exam
