A pre-retirement medical examination is required for all NOAA Corps Officers with the exception of those officers taking disability retirement or the officer's resigning. The retirement/separation medical examination is performed solely for your benefit to assist with future claims you may make with the Department of Veterans Affairs. A complete physical examination is required within 12 months preceding retirement. A minimum of 60 days and a maximum of six months is required preceding separation date. It is recommended that you schedule your medical immediately in order to meet the deadlines.
The medical examination shall follow the same requirements of an annual medical examination or a 5 year periodic examination. The following forms must be completed and submitted to the Commissioned Personnel Center along with the results of the following studies:
Laboratory Studies

CBC
-
Metabolic panel to include: Lipid Panel

Urinalysis
-
Fecal Occult Blood Test (FOBT) – beginning at age 40

PAP Smear (required every 2 years/recommended annually)
Diagnostic Studies
-
PPD (unless contraindicated)(performed annually if at sea)

Audiogram
-
Tonometry – beginning at age 40
-
Electrocardiogram (EKG) – beginning at age 40
-
Mammogram – beginning at age 40, then 44, 48, 50, 52, 54, etc.

Ocular Examination – to include: gross visual acuity (Uncorrected visual acuity must test 20/20 or better in each eye. Uncorrected vision > 20/20 must provide a copy of refractive correction)

OSHA Respirator Medical Evaluation Questionnaire

Spirometry –(at examiner’s discretion based upon OSHA Respirator Medical Evaluation Questionnaire)

Dental Examination: The "Dental Defects and Disease" section 43 of the DD Form 2808 may be used by the examining dentist in lieu of the DD FORM 2813.
Note: Please include copies of all laboratory and diagnostic test results (i.e. EKG reports, Audiograms, etc.)
